Student Athletes

MCC follows the National Junior College Athletic Association requirements for all athletes. Both new and returning athletes must submit proof of a sports physical each year, complete their student athlete section and upload all documentation to MCC's Counseling, Disability & Health Services Portal once complete. 

How to Submit Your Documentation

Student Athletes must upload their immunization records to the MCC's Counseling, Disability & Health Services Portal. Please refer to the Step-by-Step Instructions for Uploading Immunization Information.

1. Immunization Requirements

MMR Immunization Requirement

MMR Vaccine (Measles, Mumps, Rubella) – two doses given after your first birthday. These are both usually given by age five. This can also be met by proof of two measles immunizations, one mumps immunization and one rubella immunization or Proof of immunity by blood titer tests (done at any time) for Measles, Mumps and Rubella.

Meningococcal (Meningitis) Immunization Requirement

Review information about meningitis and then make an informed decision either to decline this vaccine or to show proof of vaccine(s):

  • To decline the meningitis immunization, login to the portal to complete the required form (Go to the Medical Clearances tab; Scroll to the bottom of the page; Click on clearance exemptions; Select Meningococcal ACWY or B; Complete the waiver; and Upload to the portal.)

    • Please note: Declining this vaccine ensures compliance with NYS Public Health Law but does not stop you from getting the vaccine at a later date. Students may elect to receive the vaccine at any time in the future through their healthcare provider, the Monroe County Department of Health Immunization clinic or at a local pharmacy.

  • To show proof, login to the portal to enter dates and show proof of these vaccines:
    • Meningitis ACWY (i.e., Menactra, Menveo or MenQuadfi) vaccine within the last five years or
    • Meningitis B (i.e., Bexsero or Trumenba), two dose series at any time.

    Please note: Look carefully at the dates of these vaccines because the most common reason students remain in "non-compliance" after submitting records is because their vaccine is over five years old &/or they only have had one meningitis B vaccine. If you experience any issues when attempting to upload forms, please contact the office for assistance.

Tetanus, Diptheria, Pertussis (TDAP) Immunzation Requirement

Provide proof of vaccine received within the last ten years. 

2. Athletic Physical

Athletes must submit a physical exam completed within one calendar prior to their start of athletic participation at MCC. If the athlete's physical is not completed on the MCC Preparticipation Athletic Physical Clearance Form form (also found in the Counseling, Disability & Student Health Services Portal), the physical exam submitted must indicate the athlete is cleared for athletic participation. The athletic physical is due on an annual basis for as long as a student is participating in MCC athletics.

3. Health Insurance

Students athletes are required to submit proof of current health insurance in the Counseling, Disability & Health Services Portal.

4. Athlete Waiver Consent Form

All student athletes must sign the athlete waiver consent form in the Counseling, Disability & Health Services Portal.
