Health Services oversees the initial clearance and annual reassessment process for clinical participation of all students enrolled in a health-related program.
How to Submit Your Documentation
Students must upload their immunization records to the MCC's Counseling, Disability & Health Services Portal. Please refer to the Step-by-Step Instructions for Uploading Immunization Information.

1. Immunization Requirements
MMR Immunization Requirement
MMR Vaccine (Measles, Mumps, Rubella) – two doses given after your first birthday. These are both usually given by age five. This can also be met by proof of two measles immunizations:
- one mumps immunization and one rubella immunization
or - Proof of immunity by blood titers tests (done at any time) for Measles, Mumps and Rubella.
Meningococcal (Meningitis) Immunization Requirement
Review information about meningitis and then make an informed decision either to decline this vaccine or to show proof of vaccine(s):
- To decline the meningitis immunization, login to the portal to complete the required form:
- Go to the Medical Clearances tab.
- Scroll to the bottom of the page.
- Click clearance exemptions.
- Select Meningococcal ACWY or B.
- Complete the waiver.
- Upload to the portal.
- Please note: Declining this vaccine ensures compliance with NYS Public Health Law but does not stop you from getting the vaccine at a later date. Students may elect to receive the vaccine at any time in the future through their healthcare provider, the Monroe County Department of Health Immunization clinic or at a local pharmacy.
- To show proof, login to the portal:
- Meningitis ACWY (i.e., Menactra, Menveo or MenQuadfi) vaccine within the last five years
or - Meningitis B (i.e., Bexsero or Trumenba), two dose series at any time.
- Please note: Look carefully at the dates of these vaccines because the most common reason students remain in "non-compliance" after submitting records is because their vaccine is over five years old &/or they only have had one meningitis B vaccine. If you experience any issues when attempting to upload forms, please contact the office for assistance.
Hepatitis B Immunization Requirement
- Three vaccine series or Positive Hepatitis B blood titer
or - To decline the Hepatitis B immunization at this time:
- Go to the Medical Clearances tab.
- Scroll to the bottom of the page.
- Click clearance exemptions.
- Select Hepatitis B.
- Complete the waiver and Upload to the portal.
Tetanus, Diptheria, Pertussis (TDAP) Immunization Requirement
Provide proof of vaccine within the last ten years.
Varicella (Chickenpox) Immunization Requirement
- Provide proof of two doses
or - Date of illness of chicken pox/shingles or date of positive Varicella titer.
2. Tuberculosis Screening
First year students in a health-related program are required to complete one of the following:
- PPD placement and read within six months of entry to the program
or - TSpot test results within six months of entry to the program
or - QuantiFERON Gold test results within six months of entry to the program
or - Chest X-Ray results
Please note: Clinical Laboratory Technician, Dental Hygiene and Assisting, Radiologic Technology and Surgical Technology students are required to submit an annual TB test at the time they submit their reassessment form for year two in the program.
3. Physical Exam
First year students in health-related programs are required to submit a completed Health Related Physical Exam Form prior to the start of the semester.
4. Health History
All students in a health-related program must complete a health history within the Counseling, Disability and Health Services Portal.
5. Health Major Attestation
All students in a health-related program must sign the Health Major Attestation within the Counseling, Disability and Health Services Portal.
